Users answer a series of online questions about activities like cooking, bathing, and managing medication, and the tool generates a personalised report with recommended aids, information on where to find them, and links to further support.
The questions can be completed by the individual themselves or by a friend or family member on their behalf, making the tool accessible to those who may need additional support.
