Project Support Coordinator
We are looking for an experienced project support coordinator to support the delivery of projects and our PCN Management team. The role will involve working alongside the PCN Business Manager, Digital & Transformation Lead, Clinical Director, and the wider team to achieve success in line with the PCN Network Contract DES and the PCN priorities. They will provide operational support to the PCN, leading on the administration of PCN projects and submitting relevant reports and data to the commissioners, NHS Sussex.

Advanced Clinical Practitioner (ACP)
We’re looking for a highly skilled Advanced Clinical Practitioner (ACP) to join our team. Providing support to housebound and frail patients, and supporting the Bexhill GP Practices with general triage, clinical support, and the Extended Access Service. You will be joining an established team, where we already have an ACP, A team of First Contact Practitioners (FCP’s), A Pharmacy Team, Care Co-Ordinators, and Social Prescribers.
