Chloe Trickey

I work for the Emotional Wellbeing Service and I am based within the Bexhill PCN. My role as an Access Facilitator consists of working closely with GP surgeries to identify individuals who are registered with a severe mental illness who would benefit from a physical health check. I work to engage with the individuals, complete physical health checks and to offer additional support and referrals which is relevant for the individual to make sure they are receiving the correct support.
