Kim Granville

I am the Diabetes Care Co-Ordinator for the PCN. My role is to help and support patients with their diabetes, signposting to local support groups and supporting them with their healthcare needs and to ensure they are receiving their health checks. Working within the local community to raise awareness of diabetes and to encourage patients at high risk of type 2 to access lifestyle interventions to reduce their risk of developing the condition.
